Samaritan Patient Records (MyChart)
MyChart is your electronic patient record. Your electronic health record is kept in our secure computer system and can be accessed by you, your doctors, and your providers at Samaritan Hospital and Samaritan Clinics.
MyChart gives you access to:
- Request and cancel clinic appointments
- Track visits and immunizations
- Message your providers
- Get your lab results
- View your prescriptions and request refills
- See your hospital discharge summary
- Pay your bill online

Important changes for teen MyChart users
What is happening?
If you and your family use MyChart to manage your healthcare, please be aware of the important changes coming soon to the accounts for teens age 12-17. These accounts will be disabled and will require teens to re-activate their account in person with an email or phone number:
- At an upcoming Samaritan appointment (clinic or hospital)
- Drop by a Samaritan Clinic or at the Samaritan Hospital main entrance to re-activate
Why is this happening?
- This MyChart update is being made to align with evolving federal and state guidance around teen access and privacy.
- While care delivery is not changing, teens will notice differences in how information appears in MyChart.
- Parent and guardian access through proxy accounts will continue, with expansion of what they can see as confidentiality tools are expanded.

New MyChart User Sign-Up
To sign up for MyChart, click the button below, then click SIGN UP NOW. You will be asked to enter the MyChart Activation Code that was emailed to you. If you don’t have an activation code, click SIGN UP ONLINE. If you need assistance, call the MyChart Help Desk at 833.395.2035.

As of September 12, 2020, all patient records for Samaritan Hospital and Clinics are housed under EPIC’s MyChart system.
